Doctor, are you constantly asking your staff to show you the money, read on.

A doctor not only treats patients for their various physical needs but runs a business at the same time. Doctors have large overhead expenses. They pay rent, salaries, utilities, medical supplies and many other expenses. They also have to maintain their own private family home and family expenses.

Have you noticed that single practice doctors are now mostly in the past. Multiple doctors networking together is now what is expected. This cuts down on some of the overhead.

Since HMO's and other insurance plans have taken over medical insurance for most people, the medical doctor also has to change. For instance, remember the days when you actually had ten(10) minutes with the doctor. Now you may only have five (5)minutes.

The reason is money. HMO's and other insurance companies have cut the reimbursement to doctors. The doctor has to make up the difference in revenue by seeing patients in volume.

Many older doctors are now retiring thanks to HMO's and other insurance plans. The doctors are confused and angry with the change in medical care. When they went to medical school, it did not include fighting with insurance companies for medical care for their patients.

I worked in several doctors offices over the years and I have been a patient in many. As a patient, I always observe the receptionist. I watch the interchange between the receptionist and the patients as they come up to the window to check in. Is the receptionist courteous and polite to the patient?

RECEPTIONISTS RESPONSIBILITIES:

(1) Be aware of money owed by the patient from a previous visit.

(2) Collect co pays.

(3) Make sure the patient has given you the correct insurance information. Call the insurance company and find out if the deductible has been met for the year or if there is a co pay owed.

(4) If a cash patient make sure you collect the money before the patient leaves the office.

If an arrangement has been made between the office and the patient to pay part now and part later, make sure the patient is well aware of this before they vanish forever into the sunset.

(5) Make a copy of the front and back of the insurance card.

(6) If the patient needs a referral or authorization make sure it is in the chart ahead of time. Going back and trying to get a retro leads to more paperwork and an angry patient.

(7) Make sure you have the patients correct address.

(8) Answers the phone and makes appointments.

RECEPTIONISTS multitask constantly though out the day. They have an important job. They are the spokesperson for the doctor. When a patient checks in or out of the office is when the money is collected from the patient. It can easily be missed.

MEDICAL INSURANCE BILLING:

(1) Send the claim form out correctly the first time to the medical insurance company. Take the time to check it over and make sure all information has been keyed into the computer correctly.

(2) If a mistake was made and you receive and Explanation of Benefits (EOB) back requesting medical records or whatever is needed to get the claim paid, do it immediately. This will cut down on the number of angry patient calls to you.

Billers you may not like the piles of denied Explanation of Benefits you receive back from the insurance companies but they are pure gold for getting the claim paid. The balance is usually the patients responsibility.

THE DOCTORS RESPONSIBILITY:

Sometimes, the workload is so heavy the biller gets stressed and frustrated and feels trapped. When this happens maybe, another person is needed to divide the workload.

The doctor needs to take a good look at how smoothly the office is running. How efficient is the receptionist in multitasking and collecting the money.

The doctors today need to be well informed on medical insurance and the various plans. This way the doctor can treat their patients according to what their medical plan will allow.

If during the doctors visit the patient needs an unexpected procedure etc., the procedure should be authorized with the insurance company before the procedure is performed. By treating the patient before the authorization is obtained, the doctor may not get paid for the procedure.

The explanation of benefits (EOB) from the insurance company will be denied for the reason. Not a covered benefit or no prior authorization.

The doctor must take a more active role in the running of the office and be aware of all the medical billing nightmares that pop up and why they happen.

DOCTOR - FIND THE HIDDEN MONEY SITTING IN YOUR COMPUTER:

Many times there are claims ready to be paid by the insurance company, the insurance biller must be trained to handle problem accounts.

The easiest way to find the hidden money is to run a print out of all your open accounts.

(l) Have they been followed up?

(2) Look at all the Explanation of Benefits denials that are probably stacked on a desk. This is a very good way to get a handle on what is actually happening to your money!

(3) Sometimes, all it takes is an appeal by phone or by letter to start communication once again with the insurance company.

These are some suggestions to look at in finding hidden money. This entire article can start revenue flying into your office.

The diagnosis of celiac disease is confirmed by a characteristic abnormal appearance of the small intestine under the microscope. Flattening of the normal finger like projections called villi accompanied by signs of inflammation is taken to indicate damage or injury from the storage protein gluten in wheat and similar proteins in barley and rye. The small intestine biopsy has became the gold standard for establishing the diagnosis of Celiac disease or gluten sensitive enteropathy. Before 1960 gluten withdrawal followed by improvement and subsequent worsening upon rechallenge was the diagnostic criteria.

Early in the 1960s through the 1970s the small intestine was biopsied by having people swallow a small metal capsule that was attached to a suction tube. This was used to suction up tissue into the capsule before guillotining off some tissue once the capsule was confirmed to be in the small intestine by x-ray. Now the tissue is obtained by upper endoscopy, the passage of a lighted video scope through the mouth under sedation to the small intestine, where biopsies are obtained with cupped forceps.

Celiac disease biopsy: What does the pathologist look for under the microscope?

The small intestine normally has finger like projections called villi that give it a large surface area or contact area for absorption. The villi result in a shag carpet or terry cloth towel type appearance. Lining the outside surface of each villous are intestinal cells or enterocytes that secrete mucus and absorb fluids, nutrients, minerals like iron, and vitamins like B12. On the surface of the enterocytes are digestive enzymes like lactase that digest lactose or milk sugar. At the base of the villi are crypts or circular like collections of intestinal cells.

Celiac disease biopsy: What is villous atrophy?

Normally, villi are 3-5 times longer than the crypts are tall. However, intestinal injury can result in blunting, shortening (partial villous atrophy) or complete loss of the villi and flattening (villous atrophy) of the intestinal surface. The shag carpet will have bare spots or the terry cloth towel becomes like a tee shirt. The result is lack of absorption of nutrients and water resulting in weight loss, malnutrition, and diarrhea.

Celiac disease biopsy: What if the biopsy does not show atrophy or partial atrophy?

If the villi are at least 3 times as long as the crypts are tall then no flattening or blunting of the villi is present and celiac disease becomes more difficult for the pathologist to diagnose without the history or blood test results. However, an increased number of IEL's (intra-epitheliel lymphocytes) in the setting of a positive specific blood test for celiac, symptoms and especially if supported by presence of DQ2 and/or DQ8 gene pattern, is highly suggestive of celiac disease. The difficulty comes when the blood tests for the specific tests are negative or not elevated but only the non-specific blood tests (anti-gliadin or AGA and anti-reticulin antibodies) are elevated. Also, some people with milder forms of celiac have no blood tests abnormal but have classic biopsy findings of celiac and are termed seronegative (blood test negative) celiacs.

Celiac disease biopsy: Can the biopsy be normal in celiac disease?

By definition, the biopsy has been considered the gold standard for diagnosing celiac. However, recent studies have shown that the biopsy can be normal in some people with celiac. How can this be? The pathologist reading the biopsy may interpret the biopsy as normal based on his or her bias about celiac disease, a failure to appreciate the significance of the presence of IEL's, or misuse of the older standard of >40 IEL's per 100. However, more importantly is the recent recognition that normal appearing biopsies may not be normal. Electron microscopy has revealed ultra-structural abnormalities in apparent normal biopsies of people confirmed to have celiac disease. Special stains, that include immune labeling of lymphocytes, have also confirmed increased numbers of certain types of specific lymphocytes in the villi of intestinal biopsies of people confirmed to have celiac. The bottom line is that a normal biopsy does not definitively exclude celiac disease or gluten sensitivity.

Celiac diasease biopsy: What are other possible causes of biopsy changes that mimic celiac disease?

Cows milk protein sensitive enteropathy (CMSE), viral or bacterial infections, medications (especially aspirin like arthritis medications e.g. ibuprofen etc), autoimmune enteropathy, Helicobacter pylori infection (the stomach ulcer bacteria), AIDs, common variable immunodeficiency, and lymphoma of the intestine are all possible causes of small intestine changes that may mimic celiac. However, if you have classic celiac type symptoms, a positive celiac specific antibody (anti-endomysial antibody or tissue transglutaminase antibody) and a positive response to a gluten free diet then celiac is the likely cause. The likelihood is further increased if you carry one or both of the two major genes associated with celiac disease, DQ2 and/or DQ8. Normalization of celiac specific blood tests and the biopsy after a gluten free diet confirms the diagnosis of celiac disease.

Oklahoma is a state filled with natural wonders and attractions. In fact, it has the most diverse terrain in the United States on a mile-per-mile basis, according to the EPA.

The state has four main mountain ranges (Ouachitas, Arbuckles, Wichitas, and the Kiamichis). It also has over 200 man-made lakes, giving it more than any other state. The largest lake in the state is Lake Eufaula, which covers 102,000 surface acres of water.

The EPA also recognizes Oklahoma as having 11 distinct ecoregions. These include: Western High Plains, Cypress Swamps and Forests, Ouachita Mountains, Hardwood Forest, Ozark Forest, Ozark Highlands, Caves and Prairie, Cross Timbers, Tall Grass Prairie, Central Great Plains, and Southwestern Tablelands.

Tonkawa, Oklahoma - Located in Kay County along the Salt Fork Arkansas River, Tonkawa is home to the North Oklahoma Botanical Garden and Arboretum.

A huge shark tank is the most popular exhibit, and the facility also has the largest collection of fishing tackles in the world.

Grove, Oklahoma - Grove is home to the Lendonwood Gardens, a botanical garden which is free to the public. Founded in 1995, the gardens feature a blend of American and Japanese gardening techniques.

There are over 500 varieties of daylilies, 25 types of dogwood, over 50 bonsai, 75 kinds of Garden Maples, 70 varieties of hostas, as well as a koi pond and much, much more.

Halloween contacts can add a lot of impact to your Halloween costume. People who wear corrective contact lenses often experiment with costume lenses as well. But if you dont wear contacts on an everyday basis, would Halloween costume lenses be safe and comfortable for you?

What are Halloween contacts lenses?

Halloween lenses, also called costume or theatrical lenses, are soft contact lenses with a layer of color embedded in them. Top quality costume lenses are produced by the companies that also manufacture ordinary (corrective) contact lenses. To ensure that painted special effect lenses are safe, manufacturers use special paints that dont irritate the delicate surface of your eye.

There are hundreds of different designs: cats eyes, snake eyes, spirals, white-out and black-out lenses, banshee, vampire red, and many more besides.

The majority of theatrical contact lenses are round in shape (like corrective lenses) and cover only your iris. However, there is also a type of lens, called scleral, that covers the entire visible surface of your eye. Scleral lenses are used only for special effects (not for correction) and usually cost a bit more than round ones. Sclerals are also more difficult to put in.

Are costume contact lenses comfortable?

Yes, if you select the lenses that are right for you. To make sure that your lenses are safe and comfortable, you should only go for quality brand name special effect lenses. Both brands are made from top quality materials, feel comfortable in your eyes, and come in variety of exciting designs.

Quality special effect contacts are comfortable for most people. In fact, if the lens is properly fitted, you shouldnt feel it at all. Soft contact lenses dont have any adjustment period and generally feel comfortable in your eyes straight away. However, if you dont wear contact lenses every day, you might want to order your costume contacts in advance and wear them for a few hours each day, several days before the party.

Costume lenses might get a bit less comfortable as the day goes on. The reason is that they cant be as thin as clear corrective lenses - the opaque color layer adds extra thickness - so the lens doesnt let enough oxygen get to your eyes. It is recommended not to wear them for longer than 8 hours. And you should always take them out before going to bed.

Do I need doctors prescription to get special effect lenses?

It might sound silly, but you do. Corrective or not, costume lenses are medical devices, and even though most people can wear them without problems, you should consult your eye doctor and get a prescription. Also ask the doctor how to put your costume lenses in and take them out. All brand name special effect contacts come with clear instructions, about putting them in and taking them out, but it takes a bit of practice.

Where can I get Halloween costume contact lenses?

After getting your prescription comes the time to buy your lenses. You can order them through the doctors office, but this is the most expensive option. You would save a lot if you purchased your Halloween lenses from an online contact lens retailer instead. Just make sure that the shop is reputable and specializes on contact lenses, not just Halloween costumes.

If you are wondering if it is all too much trouble, I would say go for it. Without eyes to match, a costume just doesnt look complete. And the care isnt at all difficult - all it takes is a couple of minutes. Just follow the doctors instructions.

Have you noticed the feeder site or mini-site craze?

Search engines frown on feeder sites because they (just like you and I) don't like to be 'gamed'. You can actually protect your search engine friendly reputation by not engaging in thinly-veiled feeder sites.

Feeder sites are not necessary.
